The Medicaid Perinatal Risk Tool is a web application that houses a suite of infant mortality and maternal health tools:
This tool is intended to be used by practitioners to show an estimated risk of infant mortality and preterm birth using at different stages of the patient's pregnancy (before, during, and after pregnancy). In addition, practitioners can use the calculator for shared decision making, or to guide conversations with their patients. The Risk Calculator uses the patient's Medical Record Number (MRN) to collect demographic and medical history data from Epic and then parses that information to populate the calculator. The provider will be prompted to fill in any missing information manually. When all the required data is provided, the app performs calculations based on models initially created using data from the Ohio Departments of Medicaid and Health and presents the estimated probability of the outcome along with a gauge graphic to help visualize the risk.
Currently, most providers submit a PRAF electronically by filling out the online Electronic Pregnancy Risk Assessment Form (EPRAF) via the NurtureOhio website. Some providers still submit the PRAF as a manually filled out paper form.
The PRAF-EHR pulls information directly from the patient's medical record to auto-populate much of the NurtureOhio EPRAF form, reducing the time it takes to complete a PRAF.
The SMM risk calculator functions in the same way as the IM/PTB-RC but calculates the estimated probability of severe maternal morbidity at mid pregnancy and at birth. These models were created using data from the Ohio Departments of Medicaid and Health and are intended to be used by providers to stratify patients into risk categories to help with clinical decision making.
The calculators do not save or store data, however, as part of the study, when the PRAF-EHR form is sent to Nurture Ohio, some risk data is collected and included in the form data.
The Medicaid Perinatal Risk Tool was developed by the Infant Mortality Research Partnership (IMRP), a project sponsored by the Ohio Departments of Medicaid, Higher Education, and Health. Project partners include the Ohio Colleges of Medicine Government Resource Center (GRC), led by project Principal Investigator (PI) Lorin Ranbom, and The Ohio State University (OSU) Wexner Medical Center led by co-PIs Dr. Steven Gabbe and Dr. Courtney Hebert. The OSU College of Medicine Department of Research Information Technology (RIT) develops, maintains, and runs the application.
